Community health service fear
Too many patients with chronic conditions are turning up at hospital, while community services for them are "poorly co-ordinated", a report warns.
Two thirds of services let patients go home early or only provide rehabilitation on weekdays.
That is despite half of hospital admissions taking place outside those hours, the Wales Audit Office found.
