Care system 'must change'
The system of investigating the most serious incidents of child abuse must be reformed, watchdog Ofsted has said.
It warned that a third of serious case reviews, carried out after a child dies or is seriously injured through abuse or neglect, are failing.
Chief inspector Christine Gilbert said: "Many children's services are failing to learn fast enough from the most serious cases of abuse and neglect."
