Seamen 'starved of oxygen'
Three seamen who died through lack of oxygen failed to recognise the danger of entering an enclosed space on their vessel, an accident report said.
The three went into the chain locker of a North Sea oil rig vessel to secure a rattling anchor chain, the Marine Accident Investigation Branch said.
Finlay MacFadyen, 46, from Aberdeen, Robert O'Brien, 59, of Fife, and Robert Ebertowski, 40, from Poland, all died.
